CHANGES OF PERSONAL INFORMATION
If you have moved, changed your telephone number or changed your name etc. Please inform the surgery, so we can keep our records up to date.
REPEAT DISPENSING SCHEME:
The practice is rolling out a new national scheme called repeat dispensing. This is aimed at patients who are on 4 items or less, are stable on their medication and order this regularly. The doctor or nurse will advise you if you are eligible for repeat dispensing. For more information contact Janice Davies, practice Pharmacist or Shirley Dillon, repeat dispensing co-ordinator.
